Shoulder Pain

Arthroscopic Management of Shoulder sports-related injuries (Key-Hole surgery)

Arthroscopic surgery is a new approach to treat shoulder injuries. It is a minimally invasive method that has proved to be highly effective and reduces the recovery period and scarring.

Shoulder arthroscopy is usually recommended when non-surgical shoulder pain treatments do not respond. Shoulder arthroscopy relieves the painful symptoms of many shoulder problems that damage rotator cuff tendons, labrum, articular cartilage, and other soft tissue surrounding the joint and causes shoulder pain.

The Fluid is injected into the shoulder to inflate the joint, making it easier for the orthopaedic surgeon to see all structures of the shoulder with the help of an arthroscope. After the identification of the problem, specialized instruments are used for shaving, cutting, grasping, suture passing and knot tying.arf

Shoulder arthroscopy has turned out as an effective shoulder surgery which made diagnosis, treatment and recovery of the problems faster.

Management of acute and chronic shoulder instability and dislocation

Our shoulders move in all directions more freely than other joints, which is why chances of a shoulder injury and dislocation are higher. A shoulder dislocation can result in injury of the labrum, tendons or nerves.

Irrespective of the cause and type, shoulder dislocation causes muscles to spasm and intensive shoulder pain. Orthopaedic doctor will examine the shoulder and will set your shoulder right back into its position. The process usually involves shoulder surgery. Shoulder dislocation can be treated by a closed reduction technique in which the humeral head is placed back into its socket. After this, the shoulder pain will reduce immediately.

Symptoms of shoulder instability and dislocation are :

  • Swelling
  • Shoulder pain on hand movement
  • Deformation
  • Numbness
  • Persistent feeling of shoulder looseness

Multidirectional shoulder instability can be acute and chronic, caused by repetitive strain on the shoulder. This results in recurrent shoulder pain and dislocation. An acute shoulder injury can be easily diagnosed by X-rays and ultrasound imaging. Sports physical therapy is referred initially but in cases with possibilities of deep injuries, MRI and shoulder surgery is performed.

Chronic shoulder instabilities are treated with non-invasive methods that take several months to show complete recovery. Arthroscopic shoulder surgery is a minimally invasive procedure that results in more effective and fast recovery. However, some cases might still require an open shoulder surgery. You have may be required to wear a sling for several weeks to provide support to the shoulder and hand.

Management of shoulder osteoarthritis

Degenerative joint disease or Shoulder Osteoarthritis is caused by a breakdown of cartilage in the joints. One-third of adults over the age of 60 are likely to get affected by shoulder osteoarthritis. Breakdown of cartilage results in increased shoulder pain, swelling, stiffness and friction. The condition worsens with time as arthritis cannot be reversed but there are ways to control and cure the damage.

The situation can be improved by a combination of physical therapy, aerobic exercise, oral anti-inflammatoriesand ice. Doctors may administer you sports personnel with cortisone injection that provides temporary relief but can prove to be effective for long-term pain in some cases.

In the cases where arthroscopic procedures such as cartilage resurfacing cannot prove to be effective, Arthroscopy or complete shoulder replacement is performed.

Rotator cuff tear management

Rotator cuff is a are common source of shoulder pain and dysfunction in athletes. When muscles and tendons get overworked, they cause inflammation in the area. Rotator cuff tears severity can vary from small partial thickness tears to cases with the entire torn tendon.

Rotator cuff tear happens over time as a result of aging and strain on the shoulder. It is of two types-, namely acute and degenerative tear. Acute tear generally happens due to shoulder dislocation, broken collar bone, jerking motion or heavy lifting.

Your orthopaedic doctor will examine your shoulder to check symptoms, motion and stability. Rotator cuff tear can be detected by physical examinations such as musculoskeletal ultrasound and MRI. Rotator cuff tear can be small and develop with time, Non-surgical treatment can easily treat such conditions but in case of large tears, shoulder surgery becomes the only option.

Shoulder arthroplasty

Shoulder Arthroplasty is a procedure to replace the damaged ball and socket joint responsible for shoulder pain and discomfort. The shoulder joint is replaced with a prosthesis made of polyethylene and metal components.

Shoulder Arthroplasty is performed in case of osteoarthritis, inflammatory arthritis, post-traumatic osteoarthritis and osteonecrosis involving glenoid.

Shoulder Arthroplasty can be performed in various ways depending on the condition, namely Stemmed Hemiarthroplasty, Resurfacing Hemiarthroplasty and Reverse Total Shoulder Replacement (rTSR)

Shoulder pain can be managed by physical therapy soon after the shoulder surgery is done. This will reduce the pain and help you regain strength faster. However, if you still experience shoulder pain post-surgery and have a problem then you should consult your orthopaedic surgeon as it is not normal.

Tendon transfer around the shoulder

A Rotator cuff is a group of muscles and tendons responsible for holding the shoulder joint in place and allowing the motion of the arm. Tendons get detached in some cases due to constant wear and tear giving birth to shoulder pain, irritation and weakness.

The orthopaedic surgeon manages to attach tendons to the bones with the help of sutures but in cases where the tendon is completely damaged and cannot be reattached, the surgeon has to consider “tendon transfer”. Tendon is taken from a different location and is used to repair the rotator cuff. Sutures are used to attach the transferred tendon to the required place.
